It is almost always timing. Costs are committed too early or the commercial work starts too late. A team is hired months before it is needed, or assembled in a rush the week before opening with no time to become a team at all.
The opening date is fixed the moment it is announced. Everything else has to be sequenced backwards from it — and that sequence is the whole job.
